Peptic Ulcer

This is the ulceration of the gastro-intestinal tract due to the combined action of HCl and pepsin. This occurs most commonly in either the stomach or the duodenum, but it may also occur in the oesophagus, following gastro-oesophageal reflux. It may also occur in the jejunum after partial gastrectomy.

The precise reason of the triumph of acid and pepsin over the defensive mechanism of the mucosa is unknown. Gastric ulcers occur three times more than duodenal ulcers.

Stress is generally considered to be the striking cause of peptic ulcer, especially for duodenal ulcer patients. Tobacco may be another predisposing cause although there is no positive evidence for it. The patients using asprin or long term use of pain killers are in greater danger of developing gastric ulcers. Hyperthyroidism is often associated wth an increased incidence of peptic ulcer. There is little difference between the symptomatology of patients with gastric and duodenal ulcers. The main symptom which is pain, localised to the epigastrium is periodic wth remission lasting days and weeks, months or years. The pain comes on ater in the morning, becomes worse towards evening and rarely occurs after breakfast. It may wake the patient at 2 A.M. It may be eased or aggravated by food and often improve within 10 minutes of taking alkalies e.g milk. Occasionally the patient vomits which eases the pain. Another symptom is waterbrash which seems to be caused by an increased salivary secretions. With careful dietic treatment and regulated mode of life the prognosis is quite good.
